A preliminary report on the investigation into the helicopter accident in the Cape Town city centre on 10 February 1999 has been presented to the Minister of Transport Mac Maharaj by the Commissioner for Civil Aviation Trevor Abrahams.
The report provides factual information relating to the accident, the helicopter and crew. It does not make any findings relating to the probable cause of the accident. As the helicopter was made in Russia, further investigation and conclusions relating to cause will be undertaken in conjunction with an investigator from the Russian authorities. The objective of aviation accident investigations is to establish the cause with a view to implement corrective action, and not to apportion blame.
The report notes that the helicopter was in all respects serviceable and that there is no evidence that malfunctions contributed to the accident. The crew was experienced in using the Mi8 helicopter and in carrying out underslung work. Weather was not a factor in the accident.
In terms of the requirements of section 3(10) of the Rules of the Air, 1975, approval for the operation had not been sought from the Commissioner for Civil Aviation. However, the operation was in compliance with all other requirements and a decision whether any action will be taken against Heyns Helicopters will only be made once the report is final.
The preliminary report will not be released. It must first be sent to Russia for comment. In terms of the Chicago Convention the final report, which we anticipate will be ready in the second half of this year, will be made available to the public, and be filed with ICAO and the Russian authorities.
The CAA intends to consult with helicopter operators regarding the implementation of safety standards and procedures, particularly with regard to underslung operations in order to improve safety awareness and to hopefully avoid the occurrence of similar accidents.
