The MEC for Health Dr. Benny Malakoane is currently hosting Adverse Events Presentations from various institutions in the province. The purpose of this presentations is to present cases emanating from various institutions mainly related to Maternal and Child Mortalities. The Root Cause Analyses is done for each case to determine the underlying causes of death focuses on diagnosis, treatment interventions and prognosis.

Furthermore, the Adverse Events intend to look at the bigger scope where health professionals and institutional heads are held accountable. Representative form various institutions includes: CEO's, Clinical Managers, Heads of Nursing, Obstetricians, Gynecologists, District Clinical Specialist Teams and District Health Services Managers.
